NURSE PRACTITIONER
PRIMARY FUNCTION
The Nurse Practitioner shall be licensed to practice in accordance with state law of the state where the hospice is located. The Curo Nurse Practitioner is a member of the interdisciplinary team whose primary roll is conducting face-to-face visits to assure the continued medical eligibility of the hospice patient.
JOB RESPONSIBILITIES
This is a safety-sensitive position.
Determining Medical Eligibility
Assesses physical and psychosocial status of assigned patients as outlined by Nursing Standard Operating Procedures.
Prepares documentation of the visit as indicated in the Nursing Standard Manual and the orientation instructions.
Communicates findings to the Medical Director/Associate Medical Director.
Participates in scheduled team meetings, case conferences with staff.
Provides accurate documentation with visit itineraries within three calendar days of provision of service.
Provides the necessary information to agency staff for on call report.
Responds to all identified issues as clinically appropriate and in a timely manner.
Provides medical care as needed and within the scope of practice of the Nurse Practitioner.
Quality Improvement
Integrates quality improvement results into the established plan of care.
Actively participates in the coordination of the patient?s hospice care and in the hospice?s quality assessment and performance improvement program.
Professional Development
Represents professional nursing, the Curo site and the company in a positive manner.
Completes required instructional activities within Curo and participates in instructional activities outside the Curo program.
Customer Service
Promotes a customer service oriented approach to patient care delivery.
Participates in patient conflict resolution.
May be asked to perform routine visits to patient homes and/or participate in on-call schedule (See Registered Nurse job description for signature)
QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Education: Master's or higher degree in Nursing or related field with primary focus on Nursing; Completion of a graduate level nurse practitioner education program accredited by a national accrediting body and documentation of certification by a national credentialing body.
Experience: A combination of acute-care/ hospital and home health/hospice experience.
Licenses, Certifications and/or Registration:
o Current license to practice nursing in the state of requested employment
o Current registration with the appropriate board of nursing to function in the capacity of Nurse Practitioner.
o Must have a collaborative agreement in place with a supervising physician agreeable to supervise work in hospice or must agree to enter a collaborative agreement with a Medical Director/Associate Medical Director.
o CPR Certification
o Current automobile insurance and valid driver?s license
Equipment/Tools/Work-Aids: Must be able to effectively operate nursing assessment tools, computers, facsimile equipment, copier and cell phone/beeper. Must have and maintain transportation to be used for work.
Specialized Knowledge and Skills: Knowledge of terminally ill patients and their families along with understanding of hospice; ability to apply knowledge of the special needs of hospice patient and families; knowledge of roles of all disciplines providing hospice services; excellent patient assessment skills; knowledge of community resources; good oral and written communication skills; documentation and program management; knowledge of general nursing practice; thorough knowledge of managed care principles, regulatory guidelines (i.e., Medicare, Medicaid, JCAHO, and human resource) management principles
Personal Traits, Qualities and Aptitudes: Sensitivity to the impact of life and death issues faced by individuals with terminal illness; tact in dealing with these patients and their families; ability to support others at a time of crisis. Must be able to demonstrate flexibility by performing a variety of tasks, often changing from one to another of a different nature without loss of efficiency or composure. Communicate effectively with people of all socioeconomic backgrounds. Responsible functioning in the work place, maturity, cooperation, flexibility, tact in dealing with co-workers and all members of the hospice team.
Working Conditions: Patient?s homes and/or institutional setting and automobile. Potential exposure to infections, communicable diseases, odors, blood, excreta and hazardous materials.

DIRECTION RECEIVED
Reports to the Director of Operations or designee
Actively collaborates with the supervising physician as outlined in the collaborative practice agreement on file with the state board of nursing.



Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
